近日,新華網邀請北京市西城區平安醫院院長、精神科主任醫師肖存利做客新華會客廳,從醫學和心理學的角度為網友們剖析失眠背後的原因,分享改善的建議和方法。

新華網:能否請您簡單解釋一下失眠的定義和症狀?失眠在人群中的普遍程度如何,哪些人群更容易受到失眠的困擾?
肖存利:在臨床上,我們通常定義失眠為:zaiwumingxianyouyindeqingkuangxia,yigerenruolianxuyigeyue,meizhouzhishaoyousancichuxiannanyirushuihuoweichishuimiandeqingkuang,zeshiweihuanyoushimianzhengzhuang。zaizhengzhuangshang,huanzhedeshimianbiaoxianyouhenduozhong,biruyourushuikunnan,youshuimianbijiaoqian,haiyouzaoxing。tongshi,shimianyeduihuanzhezaochenghendadekunrao。youdeshihourangtazijihennanshou,youdeshihouyingxiangdaohuanzhederenjiguanxi、工作能力、周圍環境的識別、注意力等等。

新華網:導致失眠的主要原因有哪些?是否有一些常見的生活習慣或環境因素可能引發失眠?

第(di)二(er)個(ge)常(chang)見(jian)的(de)影(ying)響(xiang)因(yin)素(su)是(shi)生(sheng)活(huo)習(xi)慣(guan)。良(liang)好(hao)的(de)睡(shui)眠(mian)衛(wei)生(sheng)習(xi)慣(guan)是(shi)確(que)保(bao)高(gao)質(zhi)量(liang)睡(shui)眠(mian)的(de)基(ji)礎(chu)。建(jian)議(yi)在(zai)昏(hun)暗(an)的(de)環(huan)境(jing)中(zhong)入(ru)睡(shui),避(bi)免(mian)在(zai)睡(shui)前(qian)受(shou)到(dao)刺(ci)激(ji),如(ru)避(bi)免(mian)飲(yin)用(yong)茶(cha)和(he)咖(ka)啡(fei)等(deng)含(han)有(you)咖(ka)啡(fei)因(yin)的(de)飲(yin)料(liao)。這(zhe)些(xie)做(zuo)法(fa)有(you)助(zhu)於(yu)為(wei)睡(shui)眠(mian)創(chuang)造(zao)一(yi)個(ge)適(shi)宜(yi)的(de)環(huan)境(jing),使(shi)人(ren)體(ti)更(geng)容(rong)易(yi)進(jin)入(ru)睡(shui)眠(mian)狀(zhuang)態(tai)。從(cong)生(sheng)物(wu)學(xue)的(de)角(jiao)度(du)來(lai)看(kan),人(ren)體(ti)的(de)副(fu)交(jiao)感(gan)神(shen)經(jing)係(xi)統(tong)在(zai)興(xing)奮(fen)時(shi)有(you)助(zhu)於(yu)促(cu)進(jin)睡(shui)眠(mian)。相(xiang)反(fan),當(dang)交(jiao)感(gan)神(shen)經(jing)興(xing)奮(fen)時(shi),人(ren)們(men)就(jiu)會(hui)感(gan)到(dao)難(nan)以(yi)入(ru)睡(shui)。咖(ka)啡(fei)因(yin)、茶堿等物質以及光刺激等都會刺激交感神經,從而幹擾睡眠。

第三個因素就是社會環境。隨著現代生活節奏的加快,夜生活變得日益豐富,這在一定程度上增加了失眠現象的發生。例如,晚上10點或11點,尤其是在節假日前,街道上燈火輝煌,社交活動頻繁,這與醫學上推薦的晚上10點到11點(dian)為(wei)入(ru)睡(shui)時(shi)間(jian)相(xiang)悖(bei)。這(zhe)種(zhong)環(huan)境(jing)使(shi)得(de)人(ren)們(men)難(nan)以(yi)按(an)時(shi)入(ru)睡(shui),因(yin)為(wei)社(she)交(jiao)活(huo)動(dong)的(de)興(xing)奮(fen)感(gan)讓(rang)他(ta)們(men)難(nan)以(yi)平(ping)靜(jing)下(xia)來(lai),不(bu)斷(duan)變(bian)化(hua)的(de)睡(shui)眠(mian)節(jie)律(lv)會(hui)幹(gan)擾(rao)我(wo)們(men)的(de)生(sheng)物(wu)鍾(zhong)。生(sheng)物(wu)鍾(zhong)是(shi)調(tiao)節(jie)身(shen)體(ti)內(nei)部(bu)各(ge)種(zhong)生(sheng)理(li)活(huo)動(dong)的(de)關(guan)鍵(jian)機(ji)製(zhi),包(bao)括(kuo)睡(shui)眠(mian)。當(dang)睡(shui)眠(mian)節(jie)律(lv)頻(pin)繁(fan)變(bian)動(dong)時(shi),生(sheng)物(wu)鍾(zhong)就(jiu)會(hui)受(shou)到(dao)幹(gan)擾(rao),導(dao)致(zhi)失(shi)眠(mian)等(deng)睡(shui)眠(mian)問(wen)題(ti)。

明確失眠原因 進行針對性治療
新華網:目前,對於失眠有哪些常見的診斷方法和評估標準?
肖存利:從(cong)症(zheng)狀(zhuang)層(ceng)麵(mian)來(lai)看(kan),當(dang)一(yi)周(zhou)內(nei)出(chu)現(xian)三(san)次(ci)以(yi)上(shang)睡(shui)眠(mian)問(wen)題(ti),並(bing)在(zai)次(ci)日(ri)產(chan)生(sheng)明(ming)顯(xian)影(ying)響(xiang)時(shi),應(ying)關(guan)注(zhu)睡(shui)眠(mian)質(zhi)量(liang)。在(zai)評(ping)估(gu)睡(shui)眠(mian)時(shi),常(chang)用(yong)的(de)方(fang)法(fa)包(bao)括(kuo)睡(shui)眠(mian)監(jian)測(ce)和(he)量(liang)表(biao)評(ping)估(gu)。睡(shui)眠(mian)監(jian)測(ce)通(tong)常(chang)在(zai)醫(yi)療(liao)環(huan)境(jing)中(zhong)進(jin)行(xing),而(er)量(liang)表(biao)評(ping)估(gu)公(gong)眾(zhong)可(ke)以(yi)自(zi)行(xing)測(ce)試(shi),例(li)如(ru)在(zai)線(xian)搜(sou)索(suo)相(xiang)關(guan)自(zi)測(ce)量(liang)表(biao)。
在醫學領域,睡眠監測中常提到的概念有快動眼睡眠相(REM)和慢波睡眠相(SWS)。這些階段涵蓋了深睡和淺睡的不同層次。大量波幅通常指示醒覺狀態,而腦電圖(EEG)和he量liang表biao是shi臨lin床chuang上shang評ping估gu睡shui眠mian質zhi量liang的de兩liang大da工gong具ju。盡jin管guan量liang表biao可ke能neng受shou到dao主zhu觀guan感gan受shou的de影ying響xiang,不bu如ru腦nao電dian圖tu精jing確que,但dan腦nao電dian圖tu的de監jian測ce需xu要yao在zai醫yi療liao係xi統tong中zhong進jin行xing。
一yi般ban來lai說shuo,如ru果guo躺tang在zai床chuang上shang超chao過guo一yi個ge小xiao時shi仍reng未wei能neng入ru睡shui,即ji是shi入ru睡shui困kun難nan。不bu同tong人ren的de入ru睡shui速su度du差cha異yi很hen大da,有you的de人ren可ke能neng很hen快kuai入ru睡shui,而er有you的de人ren則ze可ke能neng長chang時shi間jian無wu法fa入ru睡shui或huo突tu然ran驚jing醒xing,這zhe些xie症zheng狀zhuang在zai臨lin床chuang上shang都dou很hen常chang見jian。另ling外wai,一yi些xie人ren在zai淩ling晨chen3、4點鍾就醒來,這可能與多種疾病有關,包括有精神科常見的抑鬱、焦慮、輕度睡眠障礙,嚴重的甚至可能有精神疾病,如精神分裂症等。
還有一些失眠是由內科疾病引起的,比如器質性疾病、呼吸暫停綜合症、心髒科的一些疾病、疼痛等。在診斷失眠時,首先需要排除這些潛在的內科疾病因素。在確認沒有這些疾病影響的情況下,才將失眠視為一個獨立的、單純的睡眠問題。

不(bu)管(guan)哪(na)一(yi)類(lei)因(yin)素(su)引(yin)發(fa)失(shi)眠(mian),直(zhi)接(jie)用(yong)藥(yao)並(bing)不(bu)是(shi)首(shou)選(xuan)的(de)解(jie)決(jue)方(fang)案(an),改(gai)善(shan)睡(shui)眠(mian)衛(wei)生(sheng)習(xi)慣(guan)是(shi)第(di)一(yi)步(bu)。建(jian)議(yi)在(zai)睡(shui)前(qian)四(si)至(zhi)五(wu)小(xiao)時(shi)內(nei)避(bi)免(mian)飲(yin)用(yong)咖(ka)啡(fei)和(he)茶(cha),不(bu)進(jin)行(xing)過(guo)於(yu)激(ji)烈(lie)的(de)活(huo)動(dong)。同(tong)時(shi),使(shi)睡(shui)眠(mian)環(huan)境(jing)光(guang)線(xian)柔(rou)和(he),可(ke)以(yi)嚐(chang)試(shi)使(shi)用(yong)香(xiang)薰(xun)、瑜伽、聽音樂或泡熱水澡等來幫助放鬆身心,營造一個寧靜、舒適的睡眠環境,降低交感神經的活躍度,讓副交感神經興奮起來。
此外,過度關注睡眠衛生,即“睡眠焦慮”,可能導致反效果,引發失眠。當過度關注自己是否入睡,注意力集中在“睡著沒睡著”,kenenghuixianruyizhongexingxunhuan,daozhishuimianzhiliangdejinyibuxiajiang。duiyuzheleirenqun,xuyaoyindaotamenjiangzhuyilicongshuimianshangzhuanyikai,changshizuoyixiexuyaogaodujizhongzhuyilidehuodong,ruyueduyibenhuisenandongdeshu,manmanfangsongxialai。

肖存利:關於失眠和藥物成癮,許多人存在一些誤解。首先,藥物成癮通常發生在長期、daliangshiyongmouzhongyaowudeqingkuangxia,liruyitianfuyongchaoguotuijianjiliangdebaianduoleiyaowu,huozhechangshijianshiyongduanxiaoyaowurusanzuolunhemidazuolun。weibimianchengyinfengxian,zheleiyaowuyingzaizhuanyeyishengdezhidaoxiashiyong,qieshiyongshijianbuyiguochang。xianzaiyouxuduozhongxiaohuochangxiaoyaowukegongxuanze,duiyuyanzhongjiaolvdehuanzhe,yishenghuituijianshiyongchangxiaoyaowu。
